Fill online the New Patient form and immediately will receive this information to our record. Make sure to give us the list of your Medications.
STEP 3Confirm Your Appointment
You will receive a call 24 hours before your appointment to confirm it. If you need to cancel or re-schedule your visit you have to call us 48 hours before.

STEP 5Meeting us
On your first day, you will get your comprehensive dental exam with Dr. Goncalves before starting any necessary treatment. We will take X-rays and we perform an Oral Health Scan.
STEP 6Financial Details
We will check in detailed your Insurances benefits before your appointment to make sure you know what insurance cover. We also offer Financial options that fit with your family budget.
